Thus when Frieza threw him in the lava pool he should not have came back. It was proven that Goku was dead when Gohan said that he couldn't feel his fathers energy anymore.

Gohan shouldn't have transformed to Oazaru form because master Roshi had destroyed the moon before DBZ had even started due to the fact that Goku had transformed. It was proven the moon was still there because After Piccolo blew up Goku's space podGohan remained in Ozaru form untill Piccolo turned around and blew up the moon. The only way theres a possiblity of this happening is if the earth has more than one moon but since dbz is more realistic than that (on earth not planet Namek) I doubt it.

In Dragonball Gt Gohan shouldn't have been able to transform to the supersaiyan level because after the Dai Kaioshin gave him his powerup his power was boosted so high if he transformed he could destroy the earth.
